At Golden Age Management, our Life Transition Supports are designed to help seniors and caregivers navigate periods of change with guidance, compassion and practical support. Aging, caregiving, responsibilities, health changes, relocation, loss and shifting family dynamics can feel overwhelming, and our goal is to help individuals feel supported, informed and less alone through the process.

Services may include Counselling & Emotional Support, Care Coordination, System Navigation & Advocacy as well as Organizing, Downsizing, Decluttering and Transition Planning.

We take a holistic, person-centred approach that recognizes both the practical and emotional impact of life transitions while helping individuals and families move forward with dignity, clarity and confidence.

Counselling & emotional support
Our Golden Age Counselors are professionally trained in Social Service Work and Health for older adults. We understand how social isolation, grief, bereavement and changes in cognition or physical function can impact mental health. We recognize the warning signs of declining mental health and know how to act to support older adults in improving their quality of life.
A Golden Age Counselor can assist you by offering:

Care Coordinator and Intermediary Advocacy services delivered by Golden Age Managers are excellent for those who are navigating various conversations with care partners and need extra support in organizing, managing and delegating tasks. A Golden Age Care Coordinator will act as a broker and an advocate when dealing with other people and services such as family, physicians, financial advisors, lawyers, real estate agents and other organizations.

Whether you're hoping to organize, declutter and modify your home to age in place or preparing to downsize and relocate, a Golden Age Move Manager can support you through the process. We understand that 'home' has different meanings for everyone, and your reason to modify or downsize house can be a sensitive subject relating to loss, grief or change. A Golden Age Move Manager will work with you to create a project plan to limit your stress and delegate the tasks associated with your life transition.
